We provide exercise physiology, which is particularly effective in managing chronic conditions that require ongoing care and support, such as Type 2 Diabetes, cardiovascular disease and musculoskeletal conditions.
The programs:
- Chronic Disease Management – Eligible individuals can receive up to 5 allied health services per year, including exercise physiology services.
- Type 2 Diabetes Program – Individuals with Type 2 Diabetes can receive 8 group exercise classes plus an initial consultation per year.
- Aboriginal and Torres Strait Islander Health Plan – Indigenous Australians can receive an additional 5 allied health services per year, including exercise physiology services
Department of Veteran Affairs (DVA) Rockhampton & Yeppoon:
The Department of Veterans’ Affairs (DVA) support those who have served or continue to serve in the defence of the country and acknowledging their commitment and sacrifices. Through their support, entitled Department of Veterans’ Affairs (DVA) members may be referred to our team for clinically necessary treatment by their General Practitioner on a valid D904 referral form.
White card holders are eligible for clinically necessary treatment for approved conditions only.
Gold card holders are eligible for clinically necessary treatments covered by DVA’s healthcare arrangements for all health conditions.
How we treat DVA patients:
- Assess the entitled person and design an individualised exercise program that addresses their referred clinical condition
- Provide the entitled person with the skills to manage the exercise component of their treatment on their own, through exercise.
